Extension on a certain portion of medical care expenses

If it is difficult to pay for your private share (a portion) of medical costs at a medical facility due to the fact that you are a disaster victim of the major earthquake and tsunami, then you can request an extension on such payments by asking at a reception window.

You must fit the criteria of description 1 and 2 below.

1. You must be a resident of a city/town/village under the Disaster Relief Act ( Hachinohe falls under this act) and be insured.
    
2. You must be a person who reported one of the following due to the major earthquake disaster.
   1. Your house was completely or partly destroyed, burned, or other equivalent damage.
   2. The person who provides the major source of income for the household has died or is very sick.
   3. The person who provides the major source of income for the household is missing.
   4. The business where the person who provides the major source of income for the household works has ended or been suspended.
   5. The person who provides the major source of income for the household has lost their job, or currently has no income.
   6. If due to the Fukushima I nuclear accidents you were evacuated from your home because it was listed under the evacuation area announced by the Prime Minister.

Medical expenses for exams, prescriptions and visiting nursing care up until May will be extended until the end of May.

However, from among the requirements listed above, in the case of number iii it will only apply up until May until the person who provides the major source of income for the household is found, or in the case of vi it will only apply until the evacuation order is removed.

At the reception window of the medical institution, explain that you fit one of the i. ~ vi requirements listed above.

If you cannot prove that you have insurance then please tell the person at the reception window your name, date of birth, address, and how to contact you. (If you have health insurance besides the National Health Insurance or the medical care system for the later period elderly then please also tell them the name of your business establishment)

(attention1)When you apply at a medical institution, sometimes a confirmation of the contents your visit is performed by your insurance company at a later date.
